Homeownership Preservation Enrollment Package

To enroll in the NHS OC Homeownership Preservation Program you will need to read the Introduction Letter below for an overview of the program.

Before filling in the online forms you may want to review them so that you have the information required available such as your paycheck stubs, bills, tax statement and loan statements (you may need other documents not listed). The next step will be to fill in the forms below and submit them for review - the forms include: Client Counselor Contract, Borrowers Authorization and the Foreclosure Intake Form.

Don't forget to READ CAREFULLY and submit the forms as you go. The third step is to print out the last three forms for your use in collecting required documents to bring to your appointment with a NHS OC counselor and other information.

Once you have submitted this information please ALLOW 24 HOURS for WEEK DAYS and 72 HOURS on WEEKENDS for us to contact you to set up an appointment. Be sure to print out the informational pages so that you will be prepared with appropriate documentation when you meet with the Foreclosure Counselor.
